Vital ailments are status effects exclusive to Vital Strike at random. The chance of inflicting a vital ailment scales with the enemy's STR and the user's skill level. The duration of an ailment scales with the enemy's AGI and LUK. Application of an ailment refreshes the duration of all ailments on the associated enemy.

Each ailment is colloquially associated with the impairment of a vital body part, with the association attributed to the effective status, code commentary, or both. These body parts are inclusive of wrist, shoulder, waist, knee, ankle, and neck. If the enemy is inflicted with a neck impairment, Vital Strike has a DMGatk ratio of 2.00 instead of the per level value.
